Looking for an application similar to this survey app: http://www.facebook.com/apps/application.php?id=78244396481
This will have this similar look and feel.
Survey will need to have the following items:
1. 10 or so questions
2. Results will give what type of "X" they are
3. These needs to have the ability to publish to the users profile and friends home
4. Need page added for legal

I will provide content, layout, and design.
Please only bid if you are very familiar with the facebook api. Samples of work should be provided and if you already have similar application to leverage that is a plus. Timeframe must be specific since this has to be done within a certain timeframe.
